    DC Comic Questionnaire Insert Variant. This variant of Firestorm #2 contains a 4-page, 6.75-in. x 8.5-in., bound in DC customer survey form. Otherwise, the comic is identical to ordinary copies. No info is available as to how many copies of this issue or other April cover-dated issues contained the survey. However, survey copies almost never turn up, so distribution of this variant type must have been very limited. Superman guest stars in "Danger Doubled is Death." The nuclear accident that created Firestorm also effects Danton Black, who becomes Multiplex, a super-villain who can duplicate himself.
