DC Comic Questionnaire Insert Variant. This variant of Firestorm #2 contains a 4-page, 6.75-in. x 8.5-in., bound in DC customer survey form. Otherwise, the comic is identical to ordinary copies. No info is available as to how many copies of this issue or other April cover-dated issues contained the survey.
